PURCHASE AGREEMENT FOR NEW SUBSCRIBERS To purchase a complete Dish Network system from us you are confirming that you understand and agree to the following: 1. You have never signed up for Dish Network service before. 2. You agree to maintain a minimum of America's Top 60 programming for 12 consecutive months. 3. You agree to call the toll free number on the Ticket to Great TV or the professional installation certificate within 5 days of receiving your order and activate the minimum programming the day your system is installed. 4. If you purchase the Ticket to Great TV and do not comply with this purchase agreement, you hereby authorize us to charge your credit card $60 without your signature to recover our loss. 5. If you purchase a system under the Free 4 All promotion and do not comply with this purchase agreement, you hereby authorize us to charge your credit card $300 without your signature to recover our loss. Note: With the Free 4 All promotion, we are offering prices for complete satellite systems with free professional installation below our cost to new Dish Network subscribers. We rely on a commission to recover our investment and make a small profit. The first 12 months you will be given $12.50 per month in equipment credits for a single receiver system and $17 per month for a two receiver system. If you live in an apartment you will need permission from your landlord to install the dish. If you agree to the terms of this agreement to return to order form PURCHASE AGREEMENT FOR EXTRA RECEIVERS NOT COVERED ABOVE Stand alone receivers can be used to replace an old receiver, as long as maintain your current programming for at least 6 consecutive months. 1. You agree to activate the new receiver(s) within 3 weeks of receiving your order. 2. You agree to pay Dish Network $5 per month for at least 6 consecutive months, IF the receiver is used as an additional receiver, or a replacement. Note: With the DVR 510, if you subscribe to less than the Everything Pak Dish Network will charge you $4.98 per month to use the DVR feature. 3. If you do not comply with this purchase agreement, you hereby authorize us to charge your credit card an additional $60 for each stand alone receiver, without your signature. If you agree to the terms of purchase to return to order form ALL WARRANTIES ARE WITH THE MANUFACTURER NOT MICROCOM OR BUDGET SATELLITE. ALL SALES ARE FINAL.
